ABSTRACT

Antecedentes: Los tumores del estroma gastrointestinal (TEG) son los tumores mesenquimales más comunes del tracto gastrointestinal (TGI), se considera que surgen de las células de Cajal, ocurren principalmente en adultos mayores (60-65 años) y se localizan en estómago (50%-70%), intestino delgado (25%-35%), colon-recto (5%-10%) y esófago (<5%). La mayoría se presenta de manera esporádica y hasta el 70% son clínicamente sintomáticos. El diagnóstico definitivo se realiza en el estudio anatomopatológico. El pronóstico de estos tumores se determina por el tamaño, recuento mitótico y localización del tumor, clasificandose: riesgo muy bajo, riesgo bajo, riesgo intermedio y riesgo alto. Background: Gastrointestinal stromal tumors (GIST) are the most common mesenchymal tumors of the gastrointestinal tract (GI), areconsidered to arise from the Cajal cells. Theyoccur mainly in older adults, 60-65 years. Theypresent in the stomach (50%-70%), small intestine (25%-35%), colon and rectum (5%-10%)and esophagus (<5%). Most GISTs are sporadicand are clinically symptomatic. The definitivediagnosis is made through anatomic pathology study. To determine the prognosis of this type of tumors we use the size, mitotic count and location of the tumor, classified them in: very low risk, low risk, intermediate risk and high risk. ABSTRACT

Os tumores estromais gastrointestinais (GIST) são os tumores mesenquimais mais comuns do trato gastrointestinal (TGI) e representam cerca de 1% de todos os tumores do TGI. Estes tumores teriam origem nas células precursoras das células intersticiais de Cajal. 95% dos GIST expressam a proteína CD117, receptor transmembrana, que sofre várias mutações e ativações, proporcionadas pelo proto - oncogene KIT e irá desenvolver finalmente a neoplasia. Eles se desenvolvem com a mesma prevalência em homens e mulheres, geralmente acima de 50 anos. A maior incidência é observada entre a quinta e a sexta década de vida, podendo-se desenvolver em qualquer parte do TGI, contudo o estômago é a localização mais comum... A avaliação do prognóstico vai depender principalmente do tamanho do tumor e o índice mitótico. A adequada compreensão e utilização dos critérios diagnósticos e classificação dos GISTs é fundamental para o tratamento do paciente.

ABSTRACT

Smooth muscle tumors of the alimentary tract are uncommon.Cancer of small intestine comprises less than 20% of all malignant tumors. A 65-year-old male patient was admitted with complain of pain in abdomen since 7 days. He was diagnosed as a case of acute intestinal obstruction and on laparotomy an extraluminal mass was found at jejunoileal junction. Histopathology revealed a malignant gastrointestinal stromal tumor (GIST) which was confirmed by immunohistochemistry. The case is reported with review of literature and criteria for differentiation between benign and malignant tumors are enumerated.
